1. Is this a U.S. LLC with exactly one owner, and is that owner foreign (not a U.S. citizen, U.S. resident, or U.S. company)?

Answer Yes only if the LLC has one owner and that owner is not a U.S. citizen, U.S. resident, or U.S. company. If the LLC has multiple owners or elected to be taxed as a corporation, answer No.

2. Does the LLC already have an EIN from the IRS?

You’ll need the EIN before we can prepare Form 7004.

3. During 2025, did the foreign owner or another related foreign person or company pay any LLC cost personally, put money into the LLC, take money out, lend money, or receive a payment from the LLC?

Include owner-paid LLC setup or compliance costs, even if paid personally, such as formation fees, registered-agent fees, annual report fees, state renewal fees, or other maintenance costs, plus reimbursements, contributions, draws, or loans.

4. Was the LLC closed, dissolved, sold, or liquidated during 2025?

If the LLC shut down or ownership was fully sold during the year, that is usually a final-return case.

5. Other than being formed during 2025, was there any special IRS reason the LLC used something other than the normal Jan 1 to Dec 31, 2025 tax year?

Most users should answer No. Answer Yes only for special situations such as a tax-year change, consolidated group filing, or another non-standard IRS case.

6. Did the LLC have any U.S.-based staff or any physical U.S. setup, such as an office, store, warehouse, or similar operation? Or do you already know it may owe U.S. income tax?

Selling online to U.S. customers, app users, or clients by itself does not count if you had no U.S.-based staff or physical U.S. setup.
